Dr. Emily J. Kirby Offers Motiva Preservé for Less Invasive Breast Augmentation

Board-certified plastic surgeon Dr. Emily J. Kirby has announced that Kirby Plastic Surgery is now offering Motiva Preservé, an advanced breast augmentation system that uses specialized instruments to create the implant pocket while preserving natural breast tissue and support structures.

The Preservé system represents a significant addition to available breast augmentation methodologies. Rather than traditional incision and dissection techniques, Preservé uses a Channel Separator and Inflatable Balloon to shape a precise implant pocket without cutting through the supportive ligaments that help maintain natural breast position and sensation. The approach requires an incision of approximately 3 centimeters, which is smaller than the typical breast augmentation incisions.

“Preservé aligns with the tissue-preservation philosophy that has guided my practice for years,” said Dr. Emily J. Kirby, founder of Kirby Plastic Surgery and Kalos Medical Spa. “Above-the-muscle cosmetic augmentation and breast reconstruction options work with the body’s natural anatomy rather than against it. Preservé takes that approach further by giving us specialized tools designed specifically to minimize disruption to healthy tissue.”

Dr. Kirby, who has performed hundreds of breast augmentations and is known for curating Enhanced Recovery After Surgery (ERAS) protocols and regional blocks for each individual patient, noted that her early experience with Preservé has been encouraging. The technique appears well-suited for patients seeking subtle, natural-looking breast enhancement in the range of one to two cup sizes.

“What I am observing in my Preservé patients is that results look incredibly natural,” Dr. Kirby explained. “The combination of the Preservé instruments with the Ergonomix implant creates results that move and feel like natural breast tissue.”

The Preservé system exclusively utilizes the flexible Motiva SmoothSilk Ergonomix implants, which are designed to mimic the natural movement of breast tissue. These implants shift from a round to a teardrop shape based on body position and movement, accommodating up to 315 cc of volume increase. Motiva implants feature the SmoothSilk surface, which clinical data suggests may reduce the risk of capsular contracture, one of the most common breast augmentation complications.

Kirby Plastic Surgery began offering Motiva breast implants in 2024 shortly after their FDA approval, making the practice an early adopter of the technology. The addition of Preservé builds on that foundation by incorporating Motiva's purpose-built instrumentation for tissue-sparing pocket creation.

Dr. Kirby emphasized that patient selection remains important for achieving excellent results. Preservé is designed for patients seeking modest enhancement and may not be appropriate for those desiring larger volume increases or those with certain anatomical considerations. During consultations at Kirby Plastic Surgery, Dr. Kirby evaluates each patient's goals and anatomy to determine whether Preservé or another technique would best achieve their desired outcome.

“For the right candidate, this technique offers a smaller incision, less tissue trauma during pocket creation, and a short recovery that is further optimized with our existing rapid recovery protocols,” she explained.

Dr. Emily J. Kirby is a board-certified plastic surgeon and the founder of Kirby Plastic Surgery, Kalos Medical Spa, and City Surgery Center in Fort Worth, Texas. She completed a six-year integrated plastic surgery residency at the University of Kentucky and a fellowship in pediatric and craniofacial plastic surgery. She served as the first female Chief of Plastic Surgery at Texas Health Harris Methodist Hospital and has been recognized as a Castle Connolly Top Doctor for six consecutive years.
